Form 1095-C

What is Form 1095-C?

Form 1095-C is a tax document that shows information about the health insurance coverage offered to you by SLIC. It helps document whether you and your dependents were offered health coverage during the calendar year.

 

In most cases, you do not need Form 1095-C to file your federal income tax return. However, you should keep it with your tax records.

Who should request a copy of Form 1095-C?

You may want to request a copy if:

  • You were a full-time employee of SLIC at any time during the year
  • You were enrolled in SLIC’s health plan
  • Your tax preparer or advisor asks for it
  • You simply want a copy for your personal records
How do I request my Form 1095-C?

If you are a current or former full-time employee of SLIC, the IRS requires us to provide access to Form 1095-C, which includes details about the health coverage offered to you.

 

To request your Form 1095-C or to ask questions about this form, please contact us using one of the methods below. We will provide your requested form within 30 days of your request.
